CONSIDER INSTEAD:
“Possible” gram negative rod Pneumonia- will Rx Zosyn.
The term “To Cover” does not code to a diagnosis.
However, documenting a “POSSIBLE” diagnosis is acceptable language in the hospital setting.
Remember: Hospital documentation can include “possible, probable, likely or suspected” diagnoses.
