Our government (AHRQ) is tracking Patient Safety Indicators to track quality. This PSI highlights the need to document the diagnosis of ARF as POA if at all possible.

Remember to document possible/probable, likely/suspected if unsure at the time of admission.

If your patient develops ARF at any time during their hospitalization, this should be documented to accurately reflect your patient’s SOI.

New onset Acute Kidney Failure which occurs after an elective surgery that requires dialysis.

Exclusions:
Acute Kidney Failure as the Principal Diagnosis
Acute kidney Failure documented to be present on admission (POA)

Surgeons:
Document effectively to capture AKI on admission.
