Acuity
-Acute or

  • “Decompensated” or “Exacerbation” acceptable terminology as well

-Chronic or
-Acute on Chronic

Etiology
-Systolic or Heart Failure with Reduced Ejection Fraction (HFrEF)

  • -Simply documenting “EF = ________%” is not sufficient

-Diastolic or Heart Failure with Preserved Ejection Fraction (HFpEF)
-Both

*Note: Using the terminology of “dysfunction” as opposed to “failure” to describe the etiology does not accurately portray your patients’ severity of illness.
*Note: There are no codes for valvular heart failure in ICD-10. Please choose one of the above listed etiologies and then attribute it to the valvular abnormality which caused it.
