A patient’s renal function is categorized based on their estimated Glomerular Filtration Rate (GFR).
Chronic Kidney Disease (CKD) | GFR |
Stage 1 | > 90 ml/min |
Stage 2 | 60 – 89 |
Stage 3 | 30 – 59 |
Stage 4 | 15 – 29 |
Stage 5 | < 15 but no dialysis |
End Stage Renal Disease (ESRD) | on chronic dialysis (HD or PD) |
Documenting Chronic Kidney Disease without a corresponding stage does not accurately reflect how sick your patients are.
Please DO NOT describe CKD patients as having “Chronic Renal Failure” or “Chronic Renal Insufficiency” as this will not accurately report your patients’ severity of illness.
Please DO NOT document “Chronic Kidney Disease” if your patient is receiving routine outpatient dialysis therapy (hemo or peritoneal). These patients should be more accurately diagnosed as having End Stage Renal Disease (ESRD).
